This Music Withdrawal Form must be submitted by the account holder before a drop from private lesson is considered final. The school of Music at OCMD operate on a 12-month academic year. Please notify your instructor of your desire to withdraw from lessons in addition to completing this form.
Students who withdraw and re-enroll within the same academic year will be charged a $25 re-enrollment fee.
ACCOUNTS WITH TUITION PAID IN FULL
A credit or refund for lessons not received will be provided. Credits are placed on the student’s OCMD account and are available for use for 1 year from the drop date. Refund will be processed 5-10 days from the drop date to the original payment method.
ACCOUNT WITH AUTOMATIC MONTHLY PAYMENTS
This form must be received at least 48 hours befor the next scheduled monthly payment to avoid charges. Once your student’s account is reconciled, you will be charged any tuition fees still due or credited/refunded any overpayments.
